Personal Training and Self Defense Instruction
Because physical exercise and self defense instruction can be strenuous and subject the participants to risk of serious injury, we urge you to obtain a physical examination from a doctor before using any exercise equipment or participating in any exercise activity or self defense instruction.  Additionally, you agree that the use of Airsoft-type weapons, or Shocknife weapons involve risk of injury, or serious physical injury. You agree to provide your own protective gear while participating in any training. You agree that by participating in physical exercise and / or self defense training activities, you do so entirely at your own risk.  You agree that you are voluntarily participating in these activities and use of these facilities and premises and assume all risks of injury, illness, or death.  We encourage you to safeguard your personal property and are also not responsible for any loss of your personal property. 
The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people. Contact Combat Self Defense, LLC.  (Contact Combat Self Defense/CCSD) have put in place preventative measures to reduce the spread of COVID-19; however, CCSD cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending CCSD Training could increase your risk of contracting COVID-19. By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 by attending our Training and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at our Training may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Training Center employees, volunteers, and program participants and their families.
You acknowledge that you have carefully read this “waiver and release” and fully understand that it is a release of liability.  You expressly agree to release and discharge Contact Combat Self Defense and our Instructors from any and all claims or causes of action and you agree to voluntarily give up or waive any right that you may otherwise have to bring a legal action against the trainer or instructor for personal injury or property damage.
To the extent that statute or case law does not prohibit releases for negligence, this release is also for negligence.
If any portion of this release from liability shall be deemed by a Court of Competent Jurisdiction to be invalid, then the remainder of this release from liability shall remain in full force and effect and the offending provision or provisions severed here from.
By signing this release or checking the box, I acknowledge that I understand its content and that this release cannot be modified orally.
